Any health concerns or considerations that I have are listed above. I hereby give permission to the medical personnel on-site to order X-rays, routine tests, treatment, and necessary transportation for my student. In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by the event staff to secure and administer treatment, including hospitalization. In consideration for being allowed to participate in the sites programs, I agree to assume the risk of such activities, and programs and further agree to hold harmless the site and its staff members conducting the activities, and the Mattawan Grad Bash 2025 Committee from any and all claims that may result from injury or death, accident or otherwise during or arising in any way from the activities. The site and the Mattawan Grad Bash 2025 Committee are not responsible for lost, stolen, or damaged personal articles. I grant permission for pictures and video to be taken of my student during this event for the purposes of sharing only with Mattawan students and parents. I acknowledge that this General Release and Liability of the site and the Grand Bash 2025 Committee is binding on me personally and on my heirs, personal representatives, successors, and assigns.RegistrationThere is an exceptionally large expense attached to this event! Because of this, we encourage all families, if they are able, to contribute $100 to cover the costs of the event. Payment can be made through our PayPal or mail-in check. Most of the funds will be spent on transportation, entertainment, and renting a facility.
